Client and Wrap-Around Services
GHCCM Navigators work one-on-one with clients to complete a life, health and wellness
assessment that is utilized to create an action plan to help identify the basic and complex needs of the client and/or their family. Action plans coordinate in-house and community-based programs and services with the goal of helping people increase emotional, financial and spiritual stability. Action plans identify and address health issues, financial and legal concerns, unsafe or substandard living environments, housing needs, education, employment, food insecurity, mental health, substance abuse, physical or emotional abuse, disability, special needs of children or family members, community and social networks, faith and spiritual care and any other concerns. Crisis services and referrals for financial assistance for utilities and rent is offered to eligible clients.
