Eligibility
The following information must be given to Client Services for certification:
Please bring in:
- Your Social Security Card or proof of PIN number
- Picture identification
- Proof of address in Catawba County
- Verification of all household income – Pay stubs (most recent month), Social Security (current annual benefit letter), Disability (current annual benefit letter and original award date letter), Unemployment, Public Assistance, Child Support (all must be dated within the last 30 days)
- If you have zero household income, the Client Services Department will provide you with a form to fill out
- Most recent Federal Income Tax paperwork including 1040 form
- If you have ever been on Medicaid or been denied Medicaid, we will need the denial letter ending the services (service also provided by worker @ GHCCM)

*Prior to making an appointment in the Clinic or receiving services in the healthcare center, new patients must be certified by GHCCM’s Client Services Department.
